To qualify for B1 Guarantee:

1. Use the Boutique Whitening consent form and ensure it is signed.
2. Take time stamped photos prior to whitening, where shade tab and notation on the tab are clearly visible.
3. Use the super-seal custom trays from Boutique Whitening.
4. Use the full 8 syringe B1 kit at 16% concentration.
5. Take time stamped pictures after whitening has been completed, where shade tab and notation are clearly visible.
6. The photos must be no more than 12 weeks apart to claim on the guarantee.
